Sonder Steps Therapy is actively seeking a full-time Licensed Clinical Social Worker (LCSW) to join our dedicated team in Paterson, New Jersey. This position offers the opportunity to work remotely while providing essential support to clients in a hospice care setting. Work Environment

This role is predominantly remote, enabling you to balance your professional responsibilities with personal commitments while still making a meaningful impact. While working from home, you'll have the opportunity to connect with clients via telehealth services, ensuring continuity of care in a supportive environment. Additionally, you may occasionally conduct in-person visits to clients at our hospice center or within the community, enriching your practice with personal engagement.

Key Responsibilities

• Develop and implement individualized treatment plans tailored to each client's needs, ensuring they receive the support necessary to manage their mental health conditions.
• Engage with clients both online and in-person, providing psychotherapy for those suffering from major mental illnesses and emotional distress.
• Conduct thorough assessments, utilizing evidence-based screening tools to evaluate clients' needs and determine best practices for care.
• Collaborate with interdisciplinary teams, including healthcare providers, to coordinate comprehensive care plans that encompass mental, emotional, and physical health services.
• Monitor client progress, adjusting treatment plans as needed and documenting all interactions and outcomes precisely in our electronic health record system.
• Actively participate in team meetings and professional development opportunities to improve service delivery and enhance personal competencies.
• Provide crisis intervention assistance as required, ensuring timely and effective support for clients in acute distress.
• Maintain adherence to all ethical guidelines and standards set forth by professional organizations and Sonder Steps Therapy.

Patient Population / Client Focus

Your primary focus will be on clients receiving hospice care, which includes individuals facing terminal illnesses and their families. You will address their unique psychological and emotional needs during a challenging time, providing interventions that promote mental well-being and resilience while offering strategies to cope with grief and loss. The dynamic nature of this patient population requires an adaptable and empathetic approach, highlighting your commitment to serving those in need.

Qualifications

• A valid LCSW license and a Bachelor's degree in Social Work or a related field.
• A minimum of 2 years' relevant clinical experience, with a strong understanding of therapeutic frameworks.
• Excellent verbal and written communication skills, allowing for effective engagement with clients and interdisciplinary teams.
• Demonstrated ability to establish rapport and trust with clients from diverse backgrounds.
• Strong organizational skills are essential for maintaining accurate records and managing a caseload efficiently.

Preferred Experience

• Previous experience in hospice care or within similar healthcare settings is highly regarded.
• Proficiency in telehealth platforms and a familiarity with electronic health record systems.
• Skills in crisis intervention and risk assessment, ensuring the safety and well-being of clients during challenging situations.

Compensation and Benefits

Our organization values the dedication and hard work of our team members. The salary for this LCSW position ranges from USD 70,000 to 85,000 per year, commensurate with experience.
