Lead a Team That Helps People Stabilize When It Matters Most
Endeavor Health Services is growing, and we’re looking for a Critical Time Intervention (CTI) Team Leader to guide individuals through some of the most pivotal transitions in their lives—such as moving from hospitalization to community care.
In this role, you won’t just manage a team—you’ll shape a program that helps people build stability, strengthen support systems, and move toward independence. You’ll lead a multidisciplinary team that includes case managers, nursing staff, and peer specialists, all working together to provide coordinated, person-centered care.
$5,000 Sign-On Bonus
What You’ll Do
Lead the implementation of the CTI model, ensuring high-quality, consistent care
Supervise and support a multidisciplinary team, including case managers, LPN, and peer specialists
Build strong partnerships with community providers to expand client support networks
Run regular team meetings that provide direction, problem-solving, and clinical support
Ensure documentation is accurate, timely, and compliant with regulatory standards
Track program outcomes and use data to improve services and client results
Support staff growth through coaching, training, and professional development
Report on program performance to leadership with clarity and accountability
Advocate for clients’ needs within the agency and the broader community
What We’re Looking For
LMSW, LCSW, LMHC, or related clinical license
At least 3 years of supervisory experience
Valid driver’s license and reliable transportation
Ability to work occasional evenings or weekends as needed
Comfort with Microsoft Office tools
Compensation & Benefits
Salary: $68,000–$71,000 (based on experience and credentials)
$5,000 sign-on bonus

Requirements
