Key Responsibilities

• Conduct thorough assessments and evaluations to understand each client's unique mental health challenges and develop individualized treatment plans aimed at improving their overall well-being.
• Deliver one-on-one therapy sessions using evidence-based therapeutic modalities that cater to the specific needs of hospice patients and their families.
• Work collaboratively with clients to set achievable therapeutic goals, regularly review progress, and adapt treatment plans as necessary to achieve the best outcomes.
• Provide teletherapy options for clients who may have mobility or accessibility challenges, ensuring that mental health support is always within reach.
• Maintain thorough and accurate documentation of client sessions that comply with ethical, legal, and regulatory standards, safeguarding client confidentiality.
• Engage in continuous professional development, seeking opportunities to enhance clinical skills and stay abreast of the latest advancements in psychotherapy and mental health practices.

Patient Population / Client Focus

This role primarily focuses on providing mental health services to clients in a hospice care setting. You will work closely with individuals experiencing significant life changes and emotional distress associated with end-of-life care, as well as their families. Your expertise will help facilitate coping strategies, emotional support, and resources that enhance their quality of life during this challenging journey.

Qualifications

• A Master's degree or higher in Social Work from an accredited institution.
• A valid and current state license as a Licensed Clinical Social Worker (LCSW) as recognized by the relevant state board.
• No history of disciplinary actions associated with your licensure.
• Demonstrated exceptional interpersonal skills and a compassionate approach in your practice.
• Proficiency with technology and a stable Wi-Fi connection to provide uninterrupted teletherapy.

Preferred Experience

• Experience working with hospice or palliative care patients is highly desirable.
• A strong understanding of the latest evidence-based practices in psychotherapy.
• Previous experience providing telehealth services is a plus.

Compensation and Benefits
