Key Responsibilities

• Conduct thorough clinical assessments and develop tailored treatment plans using best practices and solution-focused methodologies via telehealth platforms.
• Deliver effective therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), ensuring that each session is adapted to meet your clients' unique needs.
• Maintain comprehensive and precise clinical records, ensuring compliance with all regulations, utilizing secure EHR systems to uphold the confidentiality and integrity of your clients' information.
• Collaborate with a diverse group of healthcare professionals, including psychiatrists and psychologists, to ensure a holistic approach to patient care through virtual consultations.
• Participate actively in case consultations and peer review meetings, enhancing the quality of care we provide to our patients.
• Educate patients and their families on mental health challenges, potential treatment options, and available resources, leveraging digital formats to enhance engagement.
• Keep abreast of the latest developments in mental health through continuous learning opportunities such as webinars, online courses, and virtual conferences.

Patient Population / Client Focus

In this role, you will primarily support patients in a hospice care setting, including individuals confronting life-limiting illnesses and their families. Your expertise in addressing the emotional, psychological, and social needs of this vulnerable population will be crucial as you help them navigate their unique journeys. You will employ trauma-informed care principles and provide bereavement support to help families cope during and after the care process.

Qualifications

• A Master's degree in Social Work from an accredited program.
• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in the state of Connecticut.
• A minimum of 2 years of clinical experience, particularly focused on delivering high-quality psychotherapy services.
• Demonstrated proficiency in solution-focused psychotherapy techniques, adaptable to a remote care setting.
• Strong communication and interpersonal skills, essential for building rapport and trust with clients via virtual platforms.
• Excellent problem-solving abilities and critical thinking skills, allowing for effective navigation of remote communication challenges.

Preferred Experience

• Experience providing therapy in hospice, palliative care, or geriatric settings.

Compensation and Benefits

The position offers a competitive salary range of $80,000 to $104,000 annually, reflecting your experience and contribution to our organization.
