Registered Nurse (RN) – Home Health
Location: North Princeton, New Jersey
License Required: Active New Jersey RN License
Position Overview
We are seeking an experienced Registered Nurse (RN) – Home Health to provide high-quality patient care in the North Princeton, NJ area. The ideal candidate will have strong OASIS experience and a background in high-volume home health settings. This role requires excellent clinical judgment, time management, and documentation skills.
Key Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans
- Complete OASIS assessments, Start of Care (SOC), recertifications, and discharge documentation accurately and timely
- Manage a caseload of approximately 5–6 patients per day
- Perform comprehensive patient assessments and develop individualized care plans
- Administer medications, wound care, IV therapy, and other skilled nursing interventions
- Ensure thorough and timely documentation in compliance with regulatory and agency standards
- Educate patients and families on disease management, medication management, and home safety
- Collaborate with interdisciplinary team members to coordinate patient care
Requirements
- Active New Jersey RN License (Required)
- Minimum 2 years of Home Health experience
- Strong and recent OASIS experience (Required)
- Experience working in a high-volume home health setting
- Ability to manage high documentation workload efficiently
- Excellent communication and organizational skills
Schedule Requirements
- Must be available to work 1 weekend out of every 4 weeks
Preferred Qualifications
- Experience with electronic medical records (EMR) in home health
- Strong time management and multitasking abilities
